About The Role

As a Clinical Care Partner, your role is to ensure our clients receive person-centred, high-quality care by overseeing and coordinating complex clinical and care needs through assessment, care planning, and coordination of services. Your activities will include:

  • Developing and reviewing complex care plans in collaboration with clients and multidisciplinary care teams
  • Managing budgets and facilitating multidisciplinary care activities
  • Liaising with clinical teams to monitor and respond to changing needs
  • Monitoring, reviewing, and evaluating care plans, services, and outcomes
  • Assisting clients in navigating the care system and linking them to resources
