Role Description

Are you a compassionate and clinically skilled professional looking to make a meaningful impact in aged care? Join CÜRA Community Services as a Clinical Care Partner, where you'll support older individuals to live independently at home, while ensuring care is safe, effective, and aligned with their personal goals.

As a Clinical Care Partner, you'll work in partnership with participants, families, and support networks to deliver consumer-directed aged care services. You'll provide both direct and indirect nursing support, mentor non-clinical Care Partners, and uphold clinical governance standards across the Cura Aged Care program.

Key Responsibilities

  • Deliver specialised clinical services to support functional and cognitive wellbeing
  • Provide evidence-based care aligned with best practice and clinical governance
  • Manage a caseload of participants with complex needs, developing innovative Care Plans within budget
  • Conduct clinical assessments and prescribe nursing consumables within scope
  • Guide and support Enrolled Nurses and non-clinical Care Partners
  • Liaise with GPs, Allied Health professionals, and hospital discharge teams
  • Participate in multidisciplinary case conferences and quality advisory bodies
  • Provide training and clinical oversight to office and field staff
  • Respond promptly to participant deterioration or hospital discharge
  • Promote safe working practices and contribute to quality control processes
  • Build strong relationships across the aged care sector and wider community
  • Ensure efficient use of resources within individual participant budgets
  • Lead by example as a clinical mentor and team collaborator

Qualifications

  • Registered Nurse qualifications and current AHPRA registration
  • Strong clinical and relationship-building skills
  • Experience in aged care and working with CALD communities
  • Ability to work independently and collaboratively
