The role of the Senior Clinician is to provide district-level evidence-based care that supports safe transitions of consumers into care in a community setting (or appropriate inpatient setting) based on the individual consumer's strengths, needs and wishes.
These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.

A primary goal of this position is to work collaboratively with existing inpatient and community teams, consumers, carers and other key stakeholders in the coordination of day-to-day work practices for teams working with PCLI Consumers, by supporting the assessment and progression of safe transitions of consumers into care in a community setting while promoting effective participation in mental health treatment, support and recovery.
