The Division of Aged, General Medicine and Subacute Services (AGMS) has an exciting opportunity for an enthusiastic and motivated registered nurse ready to step into a dynamic leadership role.
Our Bed-Based Transitional Care Program ward is a 30-bed ward that is dedicated to delivering exceptional care in a dynamic and nurturing environment, focused on enhancing the recovery and independence of older people post-hospitalisation and supporting their journey back into the community or on to Residental Aged Care.
We're looking for a dynamic nurse leader with proven experience and motivation with a passion and strong commitment for caring for the older person to lead our team.

Key Responsibilities You will play a pivotal role in implementing and ensuring compliance with the Strengthened Aged Care Standards, actively shaping the future of aged care at Western Health.
This includes: Leading the integration of the Aged Care Quality Standards into daily practice Collaborating across Western Health committees to ensure alignment with national aged care reforms Championing initiatives that promote safety, dignity, and quality of life for older people Driving continuous improvement through evidence-based practice and audit processes Provide effective leadership and operational management of the ward team Ensure delivery of high-quality care aligned with the principles of the Aged Care Quality Standards Position Description About Western Health Western Health is one of the largest healthcare providers in Victoria, caring for a vibrant, diverse and rapidly growing community of one million people in Melbourne's western suburbs.
