The Department of Justice and Community Safety is seeking Assessment and Transition Co-ordinator's to provide a critical role at Western Plains Correctional Centre.
About the Business Unit Western Plains Correctional Centre is a modern, maximum-security prison focussed on rehabilitation, housing up to 1248 people in custody.

About the Role The Assessment and Transition Co-ordinator (ATC) forms part of the multi-disciplinary Offender Services team, which is responsible for the development, co-ordination and delivery of a range of programs and services across several specialist portfolios: transition and reintegration, family engagement, disability, diversity and complex care.
The ATC is responsible for the administration of the Corrections Victoria Reintegration Pathway, including co-ordination and facilitation of prisoner risk and needs assessments, transition and discharge planning, parole application processes, and referrals to a range of pre and post release services.
Some of your duties will include: Support the delivery of the Corrections Victoria Reintegration Pathway in a prison environment.
Administer prisoner risk and needs assessment tools to effectively identify rehabilitation and reintegration requirements, making referrals to relevant programs and services as required.
Work collaboratively with multi-disciplinary teams to develop integrated and effective transition and other specialist support plans, including actively involving and motivating prisoners to engage in goal setting and program/service participation.

About you To be successful in this role, you will have: Demonstrated understanding of the specific transition and reintegration issues and needs of prisoners.
Demonstrated ability to administer risk and/or welfare assessment tools.
Effective stakeholder management skills.
Ability to communicate sensitively and effectively with diverse cohorts, especially Aboriginal and Torres Strait Islander communities.
Demonstrated capability to work with people who are difficult to engage individually, and in group settings.
Qualifications/requirements A tertiary qualification in an appropriate discipline is highly desirable.
Demonstrated skills and experience in human services, welfare practice or related fields are highly regarded.
