The role you could play


We are seeking a dedicated Youth Services Case Manager You will be required to deliver flexible and assertive case management response to clients with professional and consistent case management to those experiencing homelessness.

You will work collaboratively within the Youth Services Integrated Model and demonstrate commitment in actively working alongside young people and their identified support needs.

Duties

  • Deliver high quality case management to young people accessing youth services programs
  • Participate in supervision with line manager
  • All client interactions documented with SAMIS and met program expectations around case note standard
  • Individual case plans developed in collaboration with clients and include a range of interventions that address the needs or goals of clients
  • Work to the direction of case management plans to assist young people to attend appointments relating to health, wellbeing and sustainable housing
This is a permanent full-time position based in Coburg North, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

  • Tertiary Qualification in Social Sciences, Welfare or related field with a Degree Qualification preferred. Relevant experience in a Social Service environment
  • An understanding of and commitment to the mission and values of The Salvation Army
  • Compassion and professionalism in all actions and ethics
  • Strong verbal and communication skills
  • Current Victorian Driver's Licence
