About the role:
We are looking for an experienced and enthusiastic Case Manager role based at the Auspire Care office, Coburg VIC 3058 to manage a caseload of elderly clients.
Conditions of Employment
- Appointment will be in accordance with the Social, Community, Home Care and Disability Services Industry Award, Level 4 PPT
- There is a threemonth probationary period for new employees.
- Auspire Care is an equalopportunity employer.
Key Responsibilities
- Development and implementation of highquality individual care plans
- Development of supportive relationships with professional staff, suppliers, and families to ensure positive outcomes.
- Ongoing communication with clients and family members to foster partnerships and ensure choice and control of their individual care plans.
- Compliance with internal policies and procedures, statutory and contractual requirements in relation to the management of service delivery
Skills & Experience
- Diploma or equivalent qualification in Community Services
- Demonstrated experience in Case managing.
- Registered Nurse desirable
- Experience in implementing individual care plans, health, and wellbeing clients.
