We are currently seeking passionate and committed Support Workers to join our growing team, with multiple locations across Melbourne:
- South and East Melbourne

- Western Suburbs
As a Support Worker with Attainable Care, you will deliver a range of services, including but not limited to:

Provide personalized support and assistance to NDIS participants, focusing on their individual needs, goals, and preferences._
- _
Develop and maintain strong relationships with participants, their families, and other stakeholders to ensure effective communication and collaboration._
- _
Implement and regularly review individualized support plans, adjusting strategies as required to promote participants' progress and well-being._
- _
Assist with daily living activities, personal care, skill development, and community participation, promoting independence and social inclusion._
- _
Monitor and report on participants' progress and any changes in their needs, ensuring timely and accurate documentation._
- _
Adhere to all relevant legislation, regulations, and organizational policies, maintaining the highest standards of professionalism and confidentiality._
To be considered for this role, you must have:

Fluency in both English and Burmese is essential

Current NDIS Worker check, Police Check, and Working with Children's Check._

Certificate of NDIS Worker Orientation Program._
- _
A current First Aid Certificate and CPR certification._
- _
A valid Australian Driver's License & Reliable Car._
- _
A minimum of a Certificate III in Individual Support, Disability, or a related field._
- _
Experience working with people with Mental health disability and challenging behaviour._

Previous experience in a similar role, preferably within the NDIS framework.
Ability to provide 2 current/recent work-related references.
