About the role
We have an opportunity a permanent full time Support Advisor to join our Community at Home Support Team. The role will be providing support to our South, Eastern Clients the base will be in our Rockingham Hub.


In this role you'll provide leadership and guidance to a team of support and service employees to ensure support provided to consumers is of a high quality.

This position provides these duties in a relief capacity at times where the Support Advisor/s are on leave.


In this role you will be there to advise, support and monitor services to all Aged Care, Community and Home Support consumers, including both governments funded and private consumers.

Support Advisors will provide case management and guidance which includes developing effective and supportive relationships with both internal and external stakeholders to plan, promote and deliver responsive support requirements.

The Support Advisor will lead a team of direct support and service staff which includes providing guidance, mentoring and supervision and will identify and provide learning opportunities for staff to ensure support provided to consumers/carers is delivered based on a consumer directed support model incorporating the principles of wellness, independence and person centred care

About you

  • Demonstrated knowledge and experience in community care as a Coordinator / Case Manager or similar role.
  • Demonstrated knowledge of service provision, allocation and compliance with contractual obligations to ensure quality service.
  • Welldeveloped communication, and interpersonal skills
  • Demonstrated time management and organisational skills to effectively plan and prioritise work tasks.
  • Demonstrated decision making and problems solving skills to innovatively manage the provision of quality support.
  • Demonstrated ability to work effectively as a team member.
  • Demonstrated initiative and problemsolving skills
  • Welldeveloped documentation and information management skills.
  • Welldeveloped literacy and numerously skills
  • Willingness and ability to participate in the development and implementation of innovative work practices to improve the quality of consumer support

What you will need

  • Completed or working towards a Cert IV in Community Services, Aged Care or Disability
  • Demonstrated experience in Community Care as a Coordinator in a similar position
  • Understanding and experience implementing a consumerdirected care model
  • Demonstrated ability to lead and support a team
  • Covid vaccination or valid exemption is required
  • National Police Clearance (obtained within the last 6 months)
  • Current First Aid Certificate
