About the Role:


In the role of Support Coordinator, you will assist NDIS participants in coordinating and implementing a range of support services, in line with their NDIS plan to achieve their goals You will also be required to establish a strong rapport and working relationships with a range of stakeholders including People with disabilities & their family members, NDIS Local Area Coordinators, NDIS Planners, and internal and external service providers.


Key Responsibilities

  • Strengthen and enhance our participant's ability to coordinate support and participate in the community.
  • Regularly monitor our participant's NDIS plan to ensure services are being delivered within their allocated budget
  • Liaise with informal supports, mainstream services, NDIS Planners/LAC & external providers to ensure the participants are receiving quality support.
  • Build the capacity of the participants to achieve greater independence to selfdirect services and supports in the longer term.

Skills & Experience

  • Minimum of a Cert IV in Community Services
  • Minimum of 6 months as an NDIS Support Coordinator
  • High level of understanding of the National Disability Insurance Scheme (NDIS)
  • Current NSW Driver's Licence
  • Reliable Vehicle
  • Working with Children's Check (Paid)
  • Police check/NDIS Worker Screening Check
