Sydney Homeless Early Intervention Service (SHEIS) provides full case management services to young people, men, women and families where tenancies or housing are at risk and to prevent homelessness.

The role will provide holistic and client centred support to young people, individuals and families to create pathways to secure housing and reducing rental household stress.

Your key responsibilities will be to:

  • Conduct comprehensive assessments to identify client's needs, strengths and goals
  • Develop individualized case plans in collaboration with clients, focusing on housing stability, employment, healthcare and other essential needs
  • Provide ongoing case management support including advocacy, crisis intervention and referrals to community resources
  • Collaborate with internal teams and external stakeholders to coordinate services and ensure holistic support for clients
  • Maintain accurate and up-to-date documentation of client progress and service supervision
  • Participate in team meetings, trainings, and professional development activities as required

Requirements for Success

  • Bachelor's degree in social work or Diploma in community services with minimum of 3 years' experience in related field
  • Previous experience working with individuals experiencing homelessness or in a similar social setting
  • Strong interpersonal skills with the ability to build rapport with clients
  • Ability to work independently and as part of multidisciplinary team
  • Compassionate, empathetic and nonjudgmental approach to working with clients
  • Driver's license and WWCC

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people for NSW.
