About the Role


Our client has an exciting opportunity for a Case Manager to join their team in Footscray for initially a 6-week contract within the Out of Home Care division.

As the Case Manager, you will be responsible for the delivery of case management and support services to young people residing in residential and lead tenant homes.


Duties and Responsibilities

  • Develop, monitor and regularly review each placement against the LAC Care Plans and DHHS Case Plan
  • Provide support and guidance to young people experiencing difficulties in relation to education, vocational training, employment, health, family etc.
  • Record and maintain case notes within CRIS/CRISSP
  • Liaise with external stakeholders as required
  • Arrange and participate in LAC case assessments, planning and review processes
  • Work in a therapeutic manner with young people and provide guidance to Residential Care Workers and volunteers around therapeutic care

Skills and Abilities

  • Relevant tertiary qualification (Community Services, Youth Work, Social Work, Psychology etc)
  • Experience supporting high risk young people
  • Case management skills particularly in crisis intervention
  • Strong interpersonal and communication skills
  • Knowledge of trauma informed care and theory base
