We are seeking a dedicated Case Manager to join our team in Flagstaff
The role you could play
As the Case Manager, you will be responsible for providing immediate and ongoing support for clients experiencing homelessness, through the provision of intervention options through case management support including case planning. This is achieved by encouraging the client’s independence and dignity. Case Managers provide continuity of care through outreach management in the community.
What are the Open Door and Flagstaff Services?
They are residential facilities that provide temporary accommodation for men who are experiencing homelessness. The Open Door accommodates 37 single men and Flagstaff supports 64 single men. Residential services provide transitional outreach support to assist clients in achieving dignified and sustainable housing outcomes.
Duties
- Ensure client data is accurately recorded and maintained on the client database (SAMiS) to inform program planning
- Initiate contact with clients using flexible engagement strategies to build positive relationships that support case plan goals
- Participate in regular supervision in which a reflective approach is taken in relation to service objectives and casework issues
- Undertake identified training and professional development activities to support practice
- Advocate on behalf of clients with other agencies to ensure access and delivery of services
This is a permanent full-time position based in Flagstaff, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.
- Degree qualification and higher educations in Social Work, Psychology, or related field
- Relevant experience in the social service sector including case-management
- Able to demonstrate knowledge of homelessness including context, causes, models, and approaches relating to best practice
- Experience working with internal and external stakeholders to both maintain and create new connections in the sector
- Current Australian Driver’s Licence required
