Farris Care Services is seeking a highly motivated and experienced NDIS Support Coordinator to join our team.

As an NDIS Support Coordinator, you will be responsible for providing quality support coordination services to NDIS participants, their families, and carers.


  • Location: Farris Care Services, Moonee Ponds

Key Responsibilities:

  • Provide highquality support coordination services to NDIS participants.
  • Work collaboratively with the participant, their family, and other relevant stakeholders to develop, implement and review NDIS plans.
  • Build and maintain positive relationships with NDIS participants, their families, and other stakeholders.
  • Provide advocacy and support for participants to access appropriate services and supports.
  • Ensure compliance with NDIS Quality and Safeguards Commission requirements.
  • Maintain accurate and uptodate participant records in accordance with NDIS policies and procedures.
  • Contribute to the development and implementation of the Farris Care Services' service model.

Qualification and Experience:

  • Tertiary qualifications in a relevant field (e.g. social work, psychology, disability studies)
  • Demonstrated experience in support coordination or related field.
  • Knowledge of the NDIS and its legislation, policies, and procedures
  • Ability to work collaboratively with participants, families, and other stakeholders.
  • Excellent interpersonal, communication and advocacy skills
  • High level of attention to detail and organisational skills
  • Ability to work independently and as part of a team.
  • Current Driver's Licence and access to a reliable vehicle.
