About the Opportunity
Our client is a not-for-profit organisation that offers healthcare services to people of all ages.

- YWFC Case Managers offer prompt engagement and culturally sensitive, adaptable, and comprehensive case management services to young individuals facing homelessness and/or who may be expecting or already parenting.

Our approach emphasizes inventive and progressive methods to dismantle barriers and forge routes out of homelessness. Additionally, we provide assistance regarding pre
- and post-natal care, family preservation, and facilitating child reunification, ensuring alignment with the needs and aspirations of young people.

About the Role

  • Effectively oversee a caseload of young individuals through a healingcentered framework. Employ innovative approaches to disrupt disadvantage and establish pathways out of homelessness.
  • Develop sophisticated case planning and crisis interventions to address the diverse needs of young individuals, including risk assessments and safety plans.
  • Provide comprehensive case management encompassing pre and postnatal care, family preservation, and child reunification.
  • Assist young individuals in accessing ongoing accommodation options, including supported accommodation, family reconciliation, private rental, shared housing, and social or transitional housing, in alignment with their case plan.

About the you

  • A qualification in youth work, community development, or a related tertiary field, or extensive experience in the community services sector.
  • Demonstrated expertise in implementing programs and services focused on children and parents with complex needs, such as homelessness, mental health challenges, substance use issues, and family or intimate partner violence, within a case management framework.
  • Profound understanding of maternal pregnancy care pathways, frameworks for addressing family violence, key aspects of child development, and factors that may hinder a young person's development, along with insight into challenges faced by vulnerable parents in safeguarding their children's safety and wellbeing.
  • Knowledge or experience in working with atrisk young people or those experiencing homelessness, including familiarity with healingoriented approaches.
  • Strong dedication to working with young people and the ability to actively and assertively engage those facing complex needs.
