About the PRIMA Program:


The Better at Home People at Risk of Multiple Admissions (PRIMA) program is an Austin-wide approach (currently on trial) to support older people at risk of multiple hospital admissions (who have multiple chronic and complex conditions) to stay well and home and to reduce their usage of acute health services.


The program's foundational elements are:

use of machine-learning data analytics to proactively identify older people who are at considerable risk of multiple hospital admissions, and implementation of a dedicated model of care for PRIMA enrolled clients which will deliver a range of benefits for client experience and the Austin Health organisation.


About the role:

A regular day will see you:

  • Alongside fellow care manager +/
  • Geriatrician, reviewing Pro-active Patient Identifier case lists, screening for prospective PRIMA clients by way of electronic medical record review
  • Contacting prospective PRIMA clients checking eligibility, suitability, and willingness to participate in the program.
  • Enrolling consenting clients to the PRIMA Program
  • Working in partnership with the client, their families, GP and other relevant community agencies to develop and implement comprehensive individualised complex care management plans aimed at assisting the client to remain in the community where possible
  • Engaging with ED and inpatient teams during client ED presentations and ward stays
  • Contributing to weekly/fortnightly PRIMA working group meetings and multidisciplinary clinical case conferences

About you:


You will be someone with excellent communication, interpersonal and collaboration skills, with the ability to work as part of a dynamic team.

You'll be comfortable problem solving in a variety of situations, and enjoy identifying opportunities for process improvement and/or redesign.

You'll further have:

  • Nursing Registration (currently registered with Nursing and Midwifery Board of Australia) or Allied Health Professional Registration (current registration with AHPRA or eligible for membership of relevant professional association)
  • At least five years relevant and recent clinical experience
  • A clientcentred approach to service delivery
  • Welldeveloped comprehensive complex care assessment, care plan development and management skills
