We are seeking a dynamic Care Leader to join our team and play a vital role in ensuring the well-being of the children and young people in our care.
Position Overview:
As a Care Leader, you will be at the forefront of our mission to provide the highest quality of care and create a lasting legacy for every individual we serve
Your responsibilities will include:
- Reviewing and evaluating Care Plans with support from the Area Manager.
- Ensuring critical incident reporting meets mandatory and professional requirements.
- Maintaining professional shift reports, care plans, and timesheets.
- Developing and maintaining evidenceinformed case management practices.
- Ensuring compliance and licensing requirements are met to the highest standards.
- Aligning professional frameworks and approaches with care plans, case notes, and case management.
- Participating positively in professional supervision and coaching.
- Demonstrating outstanding teamwork and aligning personal and professional values.
- Engaging in continuous training and professional development.
- Upholding cultural practices and processes.
- Maintaining case management practices across the team.
Qualifications & Experience:
- Tertiary training with a relevant qualification in Behavioral Science, Counseling, Social Work, or Human Services. Desired qualification, Diploma in Child Youth and Family Intervention or relevant qualification.
- At least 2 years of experience in a similar role
- Working with Children Blue Card (Qld)
- Current Australian Driver's License (at least P2)
- First Aid Certificate plus CPR Certificate
- Department of Communities Suitability Check (QLD)
- Completion of the Hope and Healing course (10 modules)
- National Police Check (no older than 12 months)
Probation Program:
- This role comes with a 6month probation period and ongoing periodic reviews through supervision and team meetings.

Selection Criteria:
- Demonstrated skills, knowledge & experience in a professional role working with traumaaffected individuals.
- Proven experience in ethical decisionmaking and selfdirected engagement.
- Knowledge of HSQF and licensing requirements, statutory knowledge in providing services to children and young people in care.
- Strong written and verbal communication skills, including case note writing.
- Commitment to ongoing professional development and minimizing professional fatigue.
- Understanding of diversity and inclusion principles, with experience in maintaining cultural practices in the workplace.
Key Performance Indicators:
Your performance will be assessed based on Departmental Case Plan Placement Agreement, Child health passport, Educational Support Plan, Major Incidents, Critical Incidents, Concerns raised, Case Inductees, Case Inductions, and Authority to Care.
