ABOUT THE ROLE
We are looking for a Senior case worker to join the CREST program for Open Minds on a full-time basis, with a maximum term contract until 30/06/2025. This will be based at Townsville Correctional Centre and Community Corrections Office.

ABOUT THE CREST PROGRAM
The Northern Region Community Re-Entry Service Team (CREST) program provides extensive support, both in prison and post-release, for up to three months, aimed at facilitating successful reintegration into the community and preventing repeat offenses. We support clients by working collaboratively with them, their families, personal supports, and other key stakeholders, as outlined in their client plans prior to release, to enhance their quality of life through community involvement.

JOB TASKS & RESPONSIBILITES

  • Provide information, referral case work support
  • Develop, implement, and review individual holistic re-entry plans based on the clients goals and building capacity for their future success
  • Identify and assist clients in locating/accessing support services including accommodation, substance abuse, physical/mental health, family/domestic violence, work/education/training, social, cultural and spiritual support
  • Establish stakeholder relationships that encourage collaborative framework
  • Data Management, case noting and achieving monthly Key Performance Indicators

WHAT IS REQUIRED
The successful applicant will be required to hold or be willing to obtain;

  • Formal qualifications (desirable)
  • Case work and/or management experience
  • Intermediate computer skills, data entry and management
  • Excellent communication and interpersonal skills
  • Demonstrated ability to effectively manage own case load
  • Valid open driver licence
  • Right to work in Australia
  • QLD Corrective Services Security Clearance
  • Working with Children Check (blue card)
  • NDIS Worker Screening Clearance (yellow card)
  • NDIS Orientation Module certificate
