About the opportunity

Care Coordination: Oversee and coordinate the delivery of individualized care plans for residents.

Assessment: Conduct comprehensive assessments of residents' physical, emotional, and social needs.

Team Collaboration: Collaborate with multidisciplinary teams, including nurses, physicians, and social workers, to ensure holistic care.

Compliance: Ensure adherence to regulatory standards and best practices in aged care.

Family Engagement: Communicate regularly with residents and their families to address concerns and provide updates on care.

Quality Improvement: Implement quality assurance programs and monitor care outcomes to enhance service delivery.

Staff Training: Provide training and support to staff on care protocols and documentation practices.

Crisis Management: Respond to emergencies and implement appropriate interventions as needed.

Skills and Qualifications

Clinical Expertise: Strong knowledge of geriatric care principles and practices.

Communication Skills: Excellent verbal and written communication skills for effective interaction with residents, families, and staff.

Organizational Skills: Ability to manage multiple priorities and maintain accurate records.

Problem-Solving: Strong critical thinking skills to address complex care issues.

Empathy and Compassion: Demonstrated ability to provide compassionate care and support to residents.

To thrive in this role, you will have:

● Excellent clinical skill base and knowledge of legislation within Aged Care.

● Experience in leading and supervising teams of people and liaising with families.

●Excellent time management, organisational and communication skills, with the ability to adapt and problem solve.

●A compassionate, flexible and positive approach.

● The ability to nurture friendly, respectful relationships and contribute to a collaborative team environment.

You will also be required to provide evidence of, or in the process of obtaining:

● Recognised tertiary qualification and current registration as a Registered Nurse with the Australian Health Practitioners Regulation Agency (AHPRA).

●National Disability Insurance Scheme (NDIS) Clearance.

● Current COVID-19 certificate, proof of current influenza vaccination and other vaccination status consistent with public health orders.
