About the Role

We are seeking a dedicated and compassionate Registered Nurse to provide high-quality, person-centered care management and clinical assessment for home care clients in a community setting.

As a Clinical Care Coordinator, you will play a crucial role in managing client care and well-being. Your responsibilities will include specialized areas such as continence management, conducting clinical assessments and interventions, medication management, dementia care support, diabetes management, wound management, and education.

Key Responsibilities

  • Conduct clinical assessments and interventions in specialized areas such as wound management, continence management, medication management, dementia care support, and diabetes management and education.
  • Manage a small case load and ensure the safe provision of clinical care.
  • Initiate service agreements with external providers when necessary.
  • Demonstrate comprehensive knowledge of industry regulations and best practices.
  • Efficiently manage client care within Bodewell policies and procedures.
  • Mentor and support direct care staff.
  • Respond to clinical inquiries and work with health professionals to develop care plans.
  • Monitor and review care plans within budget limitations.
  • Advocate for consumers to ensure timely clinical care.
  • Coordinate Peer Review Meetings and follow-ups.
  • Network and build relationships with community resources.
  • Ensure compliance with legislation, guidelines, and standards.
  • Maintain consumer confidentiality and privacy.
  • Participate in quality reporting activities.

Qualifications

  • Minimum 3 years of experience as a Registered Nurse.
  • Current AHPRA RN registration.
  • Tertiary qualification in Nursing.
  • Right to Work in Australia.
