Permanent Part Time position

An opportunity has arisen for a Case Manager to join the Older Persons Highrise (OPHR) team. The aim of the program is to promote the health and wellbeing of isolated and vulnerable tenants by improving access to a range of
health and support services and strengthening their sense of safety and connection in the community. The OPHR team are a multidisciplinary team of social workers, health professionals and support workers that work across 5 highrises in the City of Stonington and City of Port Phillip areas.

Responsibilities:

  • Provide short-term case management to tenants.
  • Provide an initial screening assessment of presenting tenants, to identify needs, and make referrals to health services and social supports as required.
  • Complete a comprehensive common assessment and care plan for people with multiple and complex needs and limitations.
  • Plan and implement social programs on and off site within a community development framework.
  • Participate in case discussion, case conferencing and practice review.
  • Provide information, support, and advocacy, as appropriate, to tenants on a wide range of issues.
  • Manage expenditure of the allocated budget and recording of Flexible Care Funds for eligible tenants.
  • Provide support and guidance to the OPHR Support Workers.

Requirements and attributes:

  • Tertiary qualification as an Allied Health professional, registered Nurse Division 1 (eligible for registration with AHPHRA) or Social Worker (eligible for registration with AASW).
  • A minimum of three years’ post-graduate experience, preferably in a community health settings.
  • Demonstrated ability to provide services to people form diverse backgrounds with complex needs.
  • A clear understanding of the social model of health, trauma informed care and community development principles.
