About You:

We are seeking a full time passionate and experienced Senior Care Coordinator who is dedicated to making a meaningful difference in the lives of others. You should have excellent communication, customer service, negotiation, problem-solving, and decision-making skills. A respectful and welcoming attitude is essential, along with a broad understanding of the challenges faced by older people, including those with dementia, veterans, LGBTI individuals, people from culturally and linguistically diverse backgrounds, people with disabilities, and their carers/families.

About the Role:

As a Senior Care Coordinator, you will:

  • Ensure effective and high-quality person-centered care management services.
  • Guide, Supervise, and Mentor Care Consultants.
  • Collaborate with other health professionals for comprehensive care planning.
  • Advocate for clients' needs and ensure client-focused care plan services.
  • Manage budgets and financial aspects in line with client care plans.
  • Proficient in conducting and resolving investigations related to compliance and safety issues. Skilled in escalating and addressing stakeholder feedback, including compliments, hazards, incidents, and complaints.
  • Participate in marketing and program development activities.
  • Uphold BWCC's values, policies, and procedures, and ensure compliance with health and safety regulations.

To be Successful for this Role, You’ll Need:

  • Tertiary qualification in Nursing, Allied Health, Social Work/Welfare, or a related discipline.
  • Current required vaccinations and statutory declarations.
  • A current State Driver’s License.
  • Proficiency in using client data management systems like Carelink+.
  • Working with Children and NDIS Workers Screening.
  • Experience in team management and community sector knowledge is desirable.
