Our model of care is a preventative approach. Our goal is to minimise hospitalisation visits for issues that can be addressed and resolved in the home care space, given the right expertise and management. Our goal is to minimise risk and harm to the client, aligned with informed decisions and best outcomes.
The ‘Wattletree Way’ is a comprehensive, wholistic approach to expert nursing care that allows our team to journey with our clients, with the inherent understanding that human function across the ages is never ‘set and forget’ or static, that we adapt our care to the client’s needs and show up for them when and how they need us to. We deep dive into the possibilities of reversible causes and commit to supporting them via face-to-face visits and telehealth contact. ABOUT THE ROLE
On the road in your own car, mobile nursing service delivery to home & community care clients in the Melbourne Metro area. You will see clients for assessments, wound care, clinical reviews, catheter care, palliative care, dementia support, NDIS complex care management, client and carer education, and more…the list really is endless.
Depth and breadth of experience is a must, coupled with a deeply passionate, respectful, and caring wholistic approach to the Home Nursing Service environment.
Your experience will take you into the homes of clients with highly complex issues, that you identify and outline in documentation, problem-solve and implement strategies or action at the visit.
Your high-level holistic assessment and critical thinking skills, digital, written, and verbal communication skills, along with willingness to respond to client needs will be at the top of your abilities.
Your thirst to learn with an openness, passion for quality-of-life outcomes, collaboration, innovation, with a sprinkle of good humour and good spirit will be highly regarded in a candidate.
Proficiency with clinical documentation, NDIS report writing highly regarded, and organisational skills are key.
HOURS
You will be casually employed, and have availability over minimum 3-4 days per week, with flexible working hours to suit you. There is also opportunity to attend emergency call outs on weekends and after hours.

THE SUCCESSFUL CANDIDATE KEY CRITERIA:
- You will be working across Melbourne Metro areas, as far and wide as you wish to travel
- You will have a recent model smart phone, printer/scanner, laptop/desktop computer
- Software and App literacy
- Comprehensively Insured car and current unrestricted driver’s licence
- Excellent ability to deliver education face to face
- All the minimum requirements as a qualified and currently practicing Registered Nurse (including but not limited to AHPRA, Police Check, WWCC, BLS, Manual Handling, Covid training, Vaccinations, NDIS worker orientation)
- Demonstrated reliability and flexibility as a positive team member, with an exceptional and down to earth manner with clients, with proficient language and communication skills.
- Be willing to touch base at the WHG office in Diamond Creek to stock up your Nurse Kit
- Minimum 10 years current nursing experience
- Available to work across (min) 3 or 4 days per week
- You must be an Australian Citizen or Permanent Resident. Agencies or Students need not apply.
