Case Manager- Residential Care

The Role:
The Case Manager role reports to the Senior Team Leader of Residential Services. In this role, you will play an integral part of connection and outcomes for children in Residential Services.
The Case Manager role is diverse. Working and connecting alongside, Residential Service Staff Team, DFFH Case Management and Case Contracting Team and a wide range of external stakeholders.
You will hold a key position within the Care Team. Your role will include the running and chairing of the formal Care Team Meeting, being inclusive with the development of plans, actions for safety and outcome planning for the best interest of the child.
A key roll is your intervention skills in responding to the developmental needs of young people whom engage in high risk behaviours and or from trauma impacted backgrounds.
Excellent writing and verbal communication is a requirement of the role. Liaising and collaboration with the skill to connect with government and non-government stakeholders, clients, families and other networks are key to the Case Management position.
Working with the best interest and engagement of the children, you will focus on goals and achievements from client voice. In particular you will hold a focus on those young people 15+ in a leaving care space, working on goals for Independent Living Skills.
Opportunities to develop and gain further skill through training and development alongside the Residential Services Team.
Position Objectives:
Case Manager is an engaging and interaction position working directly with Residential Care young people whom are Case Managed. Working as a lead within a multidisciplinary team of internal and external stakeholders. Working around goal orientated outcomes, with a key sense of safety in developing safety and support plans for young people with high risk and trauma impacted backgrounds. High level of documentation requirements inclusive to using DFFH Service systems CRIS.To Be Successful in this Position You Will Need:
Tertiary qualifications, preferably in social work / psychology, youth work or equivalent which includes Diploma Level Qualifications. Experience within a Case Management setting. Commitment to providing quality care and have experience in leading multi-disciplinary teams.What makes this such a great role for you:
You get a salary starting at around $94,000 plus super. Salary packaging is available up to $15,900 plus an entertainment card. SCHCADS award level 5.
