Main duties of the job

We are looking for a Lead Practice Nurse who will be responsible forensuring the delivery of safe and effective nursing care to the whole practicepopulation and development of the nursing services provided at the practice.

They will provide leadership and direction to the nursing team anddevelop the clinical practice of the practice nurse team in the provision ofassessment, treatment, screening, health education services, and advise topatients. They will lead and manage the nursing resources, working closely withthe GP Partners and Management team to deliver the overall practice prioritieswhilst providing a high standard of nursing care to patients. The candidate will have skills in developingand maintaining practice policies and protocols for all nursing activity. To bethe infection control lead in the practice and ensure all staff are aware ofcurrent procedures.

They will also focus on clinical delivery such as chronic diseasemanagement, health promotion, a range of treatment room skills including woundmanagement, vaccinations and immunisations and preventative nursinginterventions to all patients.

Wound care management (including dopplers and compression bandaging, in line with current evidence-based guidelines and suture removal) Routine immunisations and vaccinations given under a Patient Group Direction (with additional paediatric vaccination certification an advantage) Cervical Screening Contraceptive pill checks / sexual health Undertake long term condition reviews with appropriate training (eg Asthma/COPD, Diabetes, CKD, Hypertension) To deliver opportunistic health promotion where appropriate Offer a holistic approach to travel health by providing comprehensive advice and risk assessments to patients prior to travel including illnesses, vaccinations and medicines, safe sex, food hygiene, sun protection Provide emergency care to patients as necessary and attend annual Basic Life Support and Anaphylaxis training Take ECGs if HCA unavailable To support the clinical team with all safeguarding matters in accordance with local and national policies To understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately Undertake the collection, storage and dispatch of pathological specimens including intravenous blood samples, swabs, smears etc and ensure the cleanliness of any equipment used, as per policy. Chaperoning and assisting, where appropriate. Assist in the implementation and management of appropriate health and safety and infection control policies, as defined in local and national polices Contributes to management of medicines and vaccines as per policy Monitor and ensure the safe storage, rotation, and disposal of medications Take responsibility for and comply with existing drugs policies regarding administration and custody of medicines and be aware of legislation and NMC advisory documents Ability to obtain and document informed consent (either verbal or written) Participate in local initiatives to enhance service delivery and patient care To delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual) Continually review clinical practices, responding to national policies and initiatives where appropriate Take prompt and appropriate action on receipt of complaints as per GP Practice Policy and participate in investigation where required Report and record all incidents according to GP Practice Policy taking appropriate action as necessary Participate in the review of significant and near-miss events applying a structured approach, root cause analysis (RCA) To undertake continuing professional development undertaking training for specific clinical tasks as well as support and participate in shared learning within the practice

Additional Responsibilities

Lead for drug medication/ controlled drugs management, both monthly checks & ordering. Ensure safe storage, rotation, and disposal of vaccines, drugs and items with an expiry date. Monitor and advise on the effective use of physical and financial resources by the nursing team Management of stock ordering Infection control lead for the practice o Support patients and colleagues in adopting sound infection control measures. o Apply infection control measures within the practice according to local and national guidelines. o Annual audit, plan & Statement actioned. Recognise the signs, symptoms, and categories of abuse and maintain awareness on procedures within the practice and the ICB Recognise and maintain awareness of appropriate referral mechanisms for family violence, children, vulnerable adults, substance abuse, and addictive behaviours. Recognise psychological needs of patients presenting with depression or suicidal tendency. Co-operate and participate in, as required, any research projects within the practice Participate in the education and training of students, including legacy nurses for new staff. Manage the annual leave and roster for the nursing staff to maintain adequate nursing cover at all times. Take Lead responsibility for Clinical area/s infection control when a CQC inspection. Ensure equipment inspection completed each year Identify and support practice development plans Support the implementation of the Clinical Governance framework within the practice including audit, development and implementation of guidelines and risk management processes. Provide effective and dynamic leadership and management skills. Lead a team towards common objectives, advise and support other team members including the identification of development needs, taking appropriate action. Annual Appraisals, regular check-in in with staff. Allocate, co-ordinate, monitor and assess the work of the nursing team and individuals Provide regular nurse team meetings & attend regular management meetings as required.
