About the Opportunity

A Care Partner is a dynamic and fulfilling role that serves as a trusted advisor, delivering high-level service to support and empower clients in making informed decisions and accessing services and funding that enable them to remain in their homes.

You will conduct comprehensive wellbeing assessments and develop individualised plans that clearly identify and address clients' goals, helping them live confidently and comfortably while maximising their quality of life. You will have assessment skills that can identify changing needs and partner with clients, families and our multidisciplinary team to respond swiftly.

This position also involves managing client feedback, providing timely updates to the Management Team and other stakeholders, and handling various administrative tasks to ensure smooth operations.

About You

We are seeking a committed and compassionate individual who is sensitive to the diverse needs of older people from various social, economic, and cultural backgrounds.

The ideal candidate will excel at building rapport with clients and their families, embodying qualities of kindness, enthusiasm, and energy. You should be ready to empower others while collaborating seamlessly with colleagues to achieve the best outcomes for our customers.

Adaptability to rapid changes is essential, alongside an organised approach to managing your caseload. We value a mindset of continuous learning and a proactive attitude toward seeking opportunities to contribute, innovate, and solve problems effectively.

Essential Criteria

  • Relevant Tertiary qualifications in the Human or Behavioural Sciences, Allied Health, Aged Care or Community Services Fields 
  • Experience in client case management 
  • National Police check  
  • Driver’s license
