We are seeking an experienced and motivated Case ManagerAssessor to join our Homecare team in the SouthMetro region to service areas from south of the river toMandurah and suburbs in between. Based at our Port Kennedy Homecareoffice this role will involve travel between all Bethanie sites andservices to ensure an active approach to meeting the HomecareService needs.
This role is being offered on a Permanent Full-Timebasis working 38 hours Monday – Friday, withflexible working arrangements available.
The Case Manager Assessor will work with the Homecare team tomanage all aspects of client onboarding for new clients as well asongoing case management for a cohort of clients prior to transferto colleagues in the case management team.
The ideal candidate will have current knowledge of the Aged CareStandards and Homecare Case Management. This role requires comfortwith both the principles of case management, as well as the abilityto pivot and manage multiple priorities to achieve positiveoutcomes for new and existing recipients of Bethanie Homecareservices.
About the Role:
Oversee and coordinate the onboarding process of new Home Care Package clients for the assigned region. Develop ongoing relationships with local networks and external agencies to strengthen access to services and supports for daily living that are important for the client’s health. Assists clients in identifying ongoing needs and goals in order to support them to remain in their own home. Responsible for ensuring all client budgets are monitored and managed in line with approved package level and individual client requirements. Be an effective member of the multi-disciplinary team to ensure all decisions and referrals for clients deliver a high standard of holistic care. Maintains reports, information and support as requested by the Regional Homecare Manager to a highly quality and in accordance to policy and procedures.About You:
Willingness to uphold the Mission, Vision and Values of Bethanie. Experience in a homecare case management environment either with clinical background or more than 3 years as a Case Manager in a similar organisation Knowledge and experience within a person-centred care context within the Aged Care industry. Knowledge of Case Management principles, Home Care Package Guidelines and Funding body. Experience developing and managing client budgets. Ability to work within a team, as well as autonomously. Excellent organisational skills and ability to manage multiple priorities. Excellent verbal and written communication skills and strong computer skills and experience with client database systems. Valid driver’s licence and access to a reliable vehicle. Current National Police Clearance, or willingness to apply for one Willing to obtain required vaccinations – COVID 19 (minimum of 3 doses) and annual flu vaccinationWorking with Bethanie:
