POSTION: Service Coordinator

We are currently seeking an experienced Service Coordinator to join our team at Inca home services as permanent part time.

The service coordinator will work closely with our NDIS participants with disability, psychosocial disability, families/carers and support workers to deliver quality, person centered, and goal focused outcomes in an efficient and safe working environment.

Responsibilities include;
- Hybrid role involving administration, supervision and support hours.
- Onboarding the new clients and ensuring that all the required tasks are fulfilled.
- Creating the Service agreements and updating these when required.
- Liase with Support coordinators, families, clients, Pharmacy, GP, OT, Physio and other relevant team members to ensure the smooth service provisions.
- Supervise site operational activities, including client service delivery, site safety, customer care and safeguarding, site inventory and issue management.
- Support the financial performance of the home service site through effective management of operating expenditure in line with budget.
- Provide support to clients aligned to their personal plans, goals and any other requirement.

As our service coordinator you will oversee daily operations in our sil home. You will lead and supervise a team of Support Workers, engage with family members and stakeholders, and work directly with participants to ensure our participants achieve their goals.

This is a full time, permanent position with 2 shifts a month working directly with the participant.

**About the Role**

The service coordinator will work closely with our NDIS participants with disability, psychosocial disability, families/carers and support workers to deliver quality, person centered, and goal focused outcomes.
- Plan, coordinate and implement agreed client care plans, service agreements and service delivery arrangements with clients and their carers.
- Experience supporting staff and clients working with complex behaviour presentation.
- OnCall; you will be part of our rotating oncall system with in management, answering calls after hours and supporting our staff.
- Conduct assessments of clients, liaising with appropriate carers, existing service providers, government agencies and other key stakeholders.
- Ensure a person-centred approach and appropriate response to all contact from clients/carers.
- Undertake risk assessments in clients homes.
- Attend care team meetings and planning meetings.
- Provide strong support and direction for disability support staff, ensuring a high level of service delivery.
- Be flexible and to grow with the service change to ensure the role meets both client and service needs.
- Maintain up to date client data and case records in line with INCA Home Service's policies and procedures and the requirements of government legislation, regulations and standards.
- Ensure that all clients have staff rostered to provide service in accordance with their service agreement with consideration to skills, experience and training of available staff and suitability to client needs.
- Ensure that all client communication, both verbal and written, is documented.
- Ensure that there are processes in place to manage changes to clients' bookings including new bookings, amendments and cancellations
- Ability to manage all rostering aspects for both Participants and employees.
- Excellent knowledge of NDIS framework.
- Excellent knowledge of the Disability Quality and Safeguard framework.
- Excellent written and verbal communication skills.

**About You**
- Minimum Diploma in Community Services, Disability, Youth and Family Intervention or similar
- Experience in the co-ordination and management of 24-hour residential care of children or young people who have experienced trauma and people with disability.
- Demonstrated capacity to manage critical issues and deal with challenging situations.
- Demonstrated experience in leadership and mentoring diverse teams.
- A sound understanding of the NDIS Quality and Safeguarding framework.
- Excellent written and oral communication skills including strong interpersonal skills, use of Microsoft Office

**Salary**: $39.26 per hour
