This is a Full Time Permanent appointment at 76 hours per fortnight.
Overview
The ART program partners with consumers who would benefit from a 10 week program based on recovery oriented psychosocial practical and therapeutic support from the MIND team, with clinical support and care coordination from a senior social worker. The team is part of the multidisciplinary Assertive Treatment Team. The social worker plans, implements, coordinates and evaluates social work services to consumers in the clinical specialty area of Mental Health using advanced social work clinical practice skills. The Senior Social Worker plans, implements and evaluates consumer safety and quality of care. Practices as a Senior Social Worker and ensures practice is in accordance to the Australian Association of Social Workers Practice Standards and Code of Ethics and SMHS policies and guidelines.
Responsibilities
Plan, implement, coordinate and evaluate social work services for consumers in the Mental Health setting.
Provide advanced clinical practice and supervision within the ART program and multidisciplinary team.
Ensure consumer safety and quality of care in accordance with standards and policies.
Operate in alignment with the Australian Association of Social Workers Practice Standards and Code of Ethics and SMHS policies.
Qualifications and Skills
Advanced social work clinical practice skills.
Ability to work within a recovery-oriented psychosocial framework.
Experience in care coordination and program delivery within mental health services.
