About the role
The Sub and Non-acute Admitted Patient (SNAP) Coordinator is responsible for ensuring all inpatients of WMH facilities will be assigned a care type that is reflective of the care they are receiving. The SNAP Coordinator liaises with the Multi-Disciplinary Team (MDT) to assess patient suitability for an Episode of Care (EOC) change and completes the relevant functional assessments and care plans.
About you
- Knowledge, experience and skills in the functional assessment of patients.
- Knowledge, experience and skills in the development of individualised patient care plans.
- Knowledge of hospital funding sources and patient care types.
- Knowledge, skills and expertise in quality improvement methodology.
- Ability to establish and maintain effective professional relationships with consumers, colleagues and stakeholders to inform and influence clinical outcomes and behaviour change.
- Ability to disseminate professional development learning to clinical professionals.
- Ability to monitor and report on clinical work practices and outcomes, as well as initiate, plan and evaluate local service improvement initiatives.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
- Vaccinated against COVID-19 as outlined in the Health Employment Directive NO.12/21and the Queensland Health Human Resources Policy B70.
