**Employment Type**: Permanent Part-Time (3-4 times a week)

**About the Role**

You'll provide specialist telephonic nutrition care and care coordination to members through comprehensive person-centred care planning or utilising an existing suite of contracted service providers.

**What would you do as our Dental Practice Manager?**
- Identify and engage Members who may benefit from chronic disease management services through outbound contact processes.
- Conduct telephonic healthcare assessments with Members.
- Provide quality, safe, appropriate information to members with health/care needs.
- Person-centred care planning, according to member goals and immediate health needs; referring people with care needs to other service providers as required; regular review to assess outcomes and modify care plans as appropriate.

**What are we looking for?**
- Accredited Practising dietitian - current unrestricted registration with Dietitian's Australia
- Computer Literacy - competency in MS Office suite
- 5+ years (full-time equivalent) experience as an Accredited Dietitian.
- Experience in Health Coaching
