**POSITION TITLE HOME CARE ADVISOR**
**QUALIFICATIONS REQUIRED: Minimum Certificate IV in Aged Care or other Allied Health qualifications**
**AWARD/GRADE/LEVEL** SCHADS Award, Level 4
**HOURS **35 hrs per week
**LOCATION **ITSOWEL Office, 21 Stewart Street, Wollongong and other locations across the Illawarra
**ROLE PURPOSE**
The **Home Care Advisor **works in partnership with consumers of Government funded Home Care Packages to coordinate high quality care and support services through internal and external providers in order to achieve the best outcomes for the consumer's quality of life.
The Home Care Advisor is responsible for the safety and quality of their own professional practice, consistent with ITSOWEL's policies and procedures, HCP guidelines and Aged Care Quality Standards.
**KEY ROLES AND RESPONSIBILITIES**
The employee's responsibilities include but are not limited to:
**Sustainability**
- Monitor client budgets and HCP expenditure, ensuring services are provided within funding levels of each HCP
- Work with accounts and finance team to ensure accurate and timely financial administration and reporting
**Administrative**
- Complete case notes after all consumer visits and phone communication
- Develop, monitor and maintain consumer support plans with a consumer directed care service delivery framework
- Ensure that all services are delivered in accordance with consumer's support plan which is supported by the MAC support plan and that it provides sufficient detail and guidance for staff to provide a comprehensive care service in line with the consumer's goals and directions
- Maintain accurate records and reports in accordance with the Aged Care Quality Standards
- Undertake (set up and follow up) administrative procedures and systems necessary for clinical care management, risk management, consumer care plan reviews and other systems as required
- Maintain and manage sub-contractors for the provision of HCP services in line with HCP Government guidelines
- Identify and control work, health and safety issues in the consumer's home
- Ensure all complaints from consumers are investigated and resolved in line with ITSOWEL policies and procedures
- Plan and action annual feedback and survey forms including follow up of improvements required
- Update Feedback Register
**Consumer Satisfaction and Impact**
- Assist and enable clients to understand their aged care funding in accordance with financial budgets
- Develop, monitor and maintain consumer support plans with a consumer directed care service delivery framework
- Ensure that all services are delivered in accordance with consumer's support plan and that they provide sufficient detail and guidance for staff to provide a comprehensive care service in line with the consumer's goals and directions
- Handling complaints and incident reports related to HCP services
**Home Care Package Client Intake and Community Reach**
- Provide an efficient, high level of customer service and helpful first point of contact for actual and potential clients and referrers
- Engage with consumers, carers, the community and service providers to improve access to ITSOWEL's HCP's
- Work effectively with other aged care services team members to ensure effective internal referrals from CHSP to HCP
- Work with executive on marketing campaigns for Home Care Packages
**Supervision, education and training of staff**
- Support and manage Care Support Workers, including planning yearly training and team meetings
- Encourage teamwork and foster good communication with Care Support Workers
**Consumer Satisfaction ** a. Assist and enable consumers to understand their aged care funding b. Together with the home care team conduct annual consumer surveys and act upon feedback

- Work collaboratively within the Organisation as well as with other external service providers and health professionals
