About the roleAs a Case Manager at VMCH, you'll be responsible for delivering high-quality, person-centred services to clients and carers across designated regions.
This role will focus on the Western and Geelong region, with Ballarat included occasionally as needed.
You'll be based at one of our VMCH Retirement Villages in Melbourne's West, such as Point Cook or Bacchus Marsh.You'll ensure all services meet funding body and organisational guidelines, applying consumer-directed care principles from initial enquiry through to ongoing support.
This role also involves community engagement and contributes to VMCH's reputation as a leader in aged care services.Key responsibilities include:Conduct comprehensive assessments and develop tailored care plansProvide advice on service options and coordinate referralsEmpower clients and carers to manage their care plans with confidenceMaintain accurate client records and ensure compliance with funding and legislative requirementsMonitor service delivery and respond to changing client needsEnsure compliance with the Aged Care Quality Standards and relevant legislationAbout youTertiary qualification in Health or a related field (nursing or allied health)Experience in case management and care planningStrong understanding of aged care reforms and consumer-directed careExcellent communication, time management, and networking skillsHold a current Victorian driver's licenceMust be open to travel across Western Victoria, Geelong, and BallaratCompletion of a National Police Check & up to date Working with Children's CheckWhat we offer?
