**Support Worker Position Description **(but not limited to):

- Support NDIS participants to engage in activities and tasks including capacity building, in home support and community access.
- Support participants to access activities and outings in the community.
- Provide direct assistance to each participant in accordance with their support/service plan and as required.
- Work in accordance with STEP AS ONE DISABILITY SERVICES medication administration policy, work health and safety procedures and client right's policies including privacy and confidentiality.
- Monitor the safety and well-being of the participant and report any concerns to the Care Coordinator or Practice Lead.
- Ensure completion of appropriate documentation such as participant progress notes, incident reports and goal attainment using the Step As One Client Management System.
- Work with directly with participants, family, relevant stakeholders and other professionals to support the participants NDIS goals and individual SMART goals.
- Undertake relevant participant specific and mandatory training modules as required to maintain NDIS practice competency requirements.
- Liaise extensively with other STEP AS ONE employees to ensure the quality and compliance requirements of service delivery.
- Actively contribute to a team environment through open communication, participation in regular Support Worker meetings, planning processes and professional development and supervision.
- Provide back up and support to other staff, as necessary.
- Participate in professional development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to work your rostered shifts, with as much notice as possible.
- Take reasonable care for his or her own health and safety.
- Report any quality and safety incident or hazards at work immediately to the Practice Lead or Care Coordinator.
- Carry out their roles and responsibilities as detailed in the relevant policies and processes.
- To transport and accompany participants to activities in the local community, including appointments.

**Qualifications - preferred**
- Cert IV Child Protection, Mental Health, Disability, Community Services or similar.
- Current NDISWC (or willing to get)
- Current National Police Check
- Working with Children Check.
- First Aid certificate and CPR (or willing to get)
- NDIS Workers Orientation (or willing to complete)
- 100 Points of ID
- Current Drivers license

**Competencies and experience**
- Experience working with children, young people or adults with complex disability who display behaviours of concern including aggression and violence.
- Genuine desire to care for other people and see them lead their very best lives.

- Demonstrated proficient written, verbal and interpersonal communication skills.
- Demonstrated ability to work with mínimal supervision, and collaboratively as a member of a multidisciplinary team.
- Strong engagement, enthusiasm, resilience and self care.
- Demonstrated commitment to professional development and month supervision.
- Good at domestic duties and assistance with personal care.
- Experience to medication administration or willing to be trained
- Supporting participants with behaviours of concern. (Desirable)
- Understanding of Behaviour Support implementation and restrictive practice (Desirable)

**Job Type**: Casual

**Salary**: $40.00 - $44.00 per hour
