**Infant Child and Family Health and Wellbeing Hub**

The Infant Child and Family Health and Wellbeing Hub is a new service involving Community Health, Early in Life Mental Health Services, and external service providers. These services will provide therapy that considers all aspects of an infant's or child's life. These services will provide wellbeing support, peer support and care coordination to help with different kinds of challenges including difficulty in attending school and mental health concerns. The Infant, Child and Family Health and Wellbeing Hub will sit alongside ELHMS - Monash Childrens and the Child and Family Program - Monash Health Community

**About the Role**

To provide paediatric Aboriginal clients in the Infant Child and Family Hub with a holistic and culturally appropriate?journey through the hub. This should consider the social determinants of health which impact the Aboriginal Community. The role will also involve assisting and supporting clients and their families to navigate the health system. You will also be working with the multi-disciplinary?team in the care of Aboriginal children and their family whilst they are in the Hub.

You will function as a member of a dynamic multidisciplinary team and will work closely with team members to promote an integrated and holistic service. This challenging role offers a mix of case work, group work and linkage work to other community services.

The Aboriginal Family Support Worker will be required to work in any clinical area at any or all campuses of Monash Health as and when required. Such a requirement may be a temporary arrangement, on an ad hoc basis, or a permanent change.
