Position Summary:

- Case Manager
- Permanent Full-time
- Based in Heidelberg
- 1x Accrued Day Off per month
- Open to Mental Health Professionals with following qualification: Registered Nurse (Div1), Clinical Psychologist, Social Work (Mental Health), Occupational Therapist
- Member of the North East Area Mental Health Service (NEAMHS) Continuing Care Service

Your new team:
CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. This role is a unique opportunity to have a real impact in meeting the needs of one of our community's most complex and vulnerable cohorts. You will form solid working-relationships within the pre-existing community teams, and foster strong therapeutic relationships with your clients. We support rehabilitation goals through the use of best evidence-based frameworks and assertive case management.

Your contribution:

- You are a team player with a commitment to quality client care
- Excellent MSE and Risk Assessment skills
- Minimum 3 years' experience in a relevant mental health setting - including acute inpatient experience
- Demonstrated knowledge of recovery and collaborative clinical practice.
- A working knowledge of the Mental Health & Wellbeing Act (2022), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines
