**The Opportunity**
The goal of the Support Coordinator is to provide non-clinical care and practical support to eligible individuals with a NDIS Plan in order to strengthen their abilities to coordinate and implement supports and participate more fully in the community.
Reporting to the Team leader, the Support Coordinator will:
- Act promptly on referrals, ensuring timely support for NDIS participants.
- Guide participants in assessing various mainstream, community, informal, and provider options which include negotiating services and prices.
- Link participants to mainstream or community services such as housing, education, transport, and health.
- Develop and maintain effective, collaborative relationships with key stakeholders that result in effective interactions, mínimal service delivery issues and appropriate referrals. Key stakeholder include but are not limited to:
- NDIS Service providers
- Clients and their families
- Carers/Guardians
- Act as the primary contact for service issues, complaints, major changes and plan reviews.
- Maintain client data base which may include but not limited to, soft and hard copy client files,, progress notes and activities, NDIS billing costs etc.
- Develop comprehensive service agreements and create service bookings with preferred providers.
- Coordinate necessary assessments to determine funding requirements.
- Determine budgets for each support type and collaborate with plan managers for fund allocation.
- Establish appropriate claim categories and allocate funds correctly in consultation with plan managers.
- Strengthen participants' capacity to coordinate supports, self-direct, and manage supports effectively.
- Assist participants in preparing for plan reviews, evaluating goal achievement, and identifying solutions to challenges.

**Qualifications and Skills**:
- Tertiary qualifications in disability services, social work, or a related field with a minimum of Certificate IV.
- 2+ years of experience in case management and/or providing professional and flexible assistance people with disabilities.
- Probity checks which include an NDIS Workers screening check and QLD/NSW Working with childrens checks
- Previous experience in a support coordinator role within the NDIS preferred but not essential.
- Passion for working with vulnerable clients across a broad age and different cultural groups.
- Ability to network and develop effective working relationships.
- In-depth understanding of the NDIS Participant Portal (myplace).
- Strong communication, negotiation, and relationship management skills.
