Working across our Claremont and Cottesloe Offices, St Louis Care at Home, is seeking an experienced and compassionate Care Partner with aged care or community care experience to work Wednesday, Thursday and Friday (with potential for growth).

Your key responsibilities will include, performing client onboarding, care plans, reviews, daily care management, including closely liaising with families, health professionals and record keeping, for Home Care package, Support at Home and private clients, including monitoring and reviewing client general health and wellbeing. A shared after-hours on-call is required for a minimum of one week per month.

**Care Partner Responsibilities**:

- A demonstrated passion for working with older people, delivering person-centred care, reablement and wellness objectives for each client.
- Sound knowledge and practice of home care relevant guidelines, consumer rights, aged care standards to deliver best practice care and compliance with the Aged Care Act (legislative regulations).
- Undertake ongoing identification of client's health and care needs to ensure they are receiving optimal care and support services matched to their personal goals, preferences and needs.
- Negotiate, develop and review care plans with the client and their chosen support person, representative to ensure the client's needs come first, including dignity of risk.
- Partner clients to deliver both timely and cost-effective home care support services on budget.
- Liaise/refer to external service providers, medical and allied health professionals.
- Provide information on aged care options for clients and their families so they understand how the Australian aged care system works, including service fees, and the clients contribution.
- Follow up on clients returning home from hospital or respite care, to ensure all care needs have been identified, implemented and documented.
- Liaise with My Aged Care and Department of Human Services, as required.

**Care Partner Requirements**:

- Tertiary qualification in related field (Social Work, Nursing, Physiotherapy, OT)
- Professional registration.
- Knowledge of Home Care Packages, Support at Home and Strengthen Aged Care Quality Standards
- A motor vehicle driver's license and appropriately insured vehicle.
- National Police Clearance (Less than 1 year old).
- Right to work in Australia.
- Current First Aid Certificate
- Minimum two years experience in aged care/community care
- Experience with industry recognised Care Management Software System desired.

**Job Types**: Part-time, Casual

Pay: $42.00 - $52.00 per hour

Expected hours: 20 - 30 per week
