The National Disability Insurance Scheme (NDIS) helps people living with a broad range of physical, intellectual and psychosocial impairments in the form of funded packages. This means money they are able to use to pay for support and services they might need to go about daily life. Utilising funding can be tricky, especially for those that might face barriers in accessing, communicating and coordinating the services and support they need. This is where a Support Coordinator comes in

A Support Coordinator works directly with people living with disability and their families, helping them maximise the funding that the NDIS has provided. They play a pivotal role around the person receiving NDIS funding ensuring that funds are used and allocated correctly, connecting and coordinating people and services, providing feedback between parties, and providing reports to the NDIA.

Success in this role is helping someone utilise their funding in a way that they are able to meet all their goals and aspirations personally and as outlined in their NDIS plan.

**Tell me more about the role**

Currently, we have an opportunity for a full-time (Monday - Friday) permanent, Support Coordinator in our Caboolture office, in North Brisbane. This role is paid at a Level 4 under the SCHADS Award, which equates to $77,578 + 10.5% Superannuation per annum.

In this role your day-to-day responsibilities will include but are not limited to:

- Supporting and communicating directly with people living with disability, their families and other support networks to provide information and understanding of their NDIS plan
- Identifying the most appropriate services within the community to suit individual needs
- Advocating for, and supporting individuals to communicate their goals and aspirations to their stakeholder team and the National Disability Insurance Agency
- Attending home and community visits to check in on how things are going, and build relationships and networks
- Undertaking general case management of a person's file including official notes and reports
- Managing and monitoring a person's budget to ensure they are utilising all the funds available to them as per their NDIS Plan

Some of these points may sound complicated, however don't let this put you off -our comprehensive training and support will ensure you have the best chance of success in the role

Any experience you may have in health or community services, family care, other human services or in case management, is a great base to start as a Support Coordinator. However, we also value people who are passionate about assisting others in this field, are organised and process-driven, have solid communication and networking skills, are keen to learn, grow and make a difference
