Moreton ATSICHS operates a growing network of healthcare clinics for local Aboriginal and Torres Strait Islander people. Our model of care provides holistic health and wellbeing services to support our Community across their lifespan. We bring together primary care, allied health, dental, mental health and wellbeing, maternity and early childhood, disability services, specialist services, and aged care as one team focussed on clients' individual needs and goals.
MATSICHS Social Health integrates alcohol and other drug, mental health and suicide prevention services within the framework of the primary health care service model. The primary objective of the team is to deliver community and clinic-based treatment and support programs that facilitate client led recovery.
About the role
The purpose of the position is to provide high level coordination supports to Indigenous clients who experience severe and persistent mental health challenges and to work effectively with their support network (social support agencies, carers, families, friends and community) to identify and access appropriate services and support to meet their needs and support independent living in the community The position will also work collaboratively with a range of organisations and health care providers to coordinate, improve, change and integrate services to meet the needs of clients.
Within this role, you will be required to:
- Manage a personal caseload of clients (10 – 15 clients) with chronic and complex care coordination needs, including linking clients with longer term NDIS supports if required.
- Working with clients to connect to community services with a focus on strengthening social, educational and vocational skills.
- Actively working with clients to remove barriers and build capacity to manage day to day activities.
- Connect clients with clinical care as needed and support their engagement.
- Participating in group program delivery in collaboration with the broader multidisciplinary team.
- Work as part of the Staying Deadly and MATSICHS primary care teams to implement systems, procedures, and service offerings that improve the quality of service to Aboriginal and Torres Strait Islander Staying Deadly clients.
- Comply with data collection and reporting requirements.
- Other duties consistent with your position where require and/or requested by your manager with consideration to your level of skill, competence, limitations and training.
About you
- Minimum Cert IV in mental health, AOD or other human services.
- Minimum 2 years demonstrated experience in an equivalent role.
- Current C Class Drivers Licence (QLD)
- Current Criminal History Check
- Current Working with Children Clearence (QLD Blue Card) – required prior to your commencement
