About the roleWe are seeking compassionate, experienced, and proactive Support Workers to join our SIL team in Caloundra area QLD.
In this role, you'll work directly with participants, particularly individuals living with Acquired Brain Injury (ABI), Dementia, Down Syndrome as well as other intellectual and physical disabilities supporting them to live independently while enhancing their quality of life.
This is a rewarding opportunity to make a meaningful difference in the lives of others.What you'll be doingDeliver high-quality care tailored to participants in a professional mannerImplement behaviour support strategies and assist participants to achieve personal goalsWork collaboratively with healthcare professionals, families, and support coordinatorsMaintain accurate documentation, reports, and participant progress notesAssisting clients with daily living activities such as personal care, meal preparation, and light housekeepingProviding social and emotional support to clients, engaging them in meaningful activities and outingsAdvocating for client needs and communicating effectively with relevant healthcare professionalsMaintaining accurate records and documentation related to client careAdhering to all relevant policies, procedures, and regulatory requirementsWhat we're looking forWe are looking for Support Workers who are passionate about making a difference and have experience working with participants with complex needs.Previous experience in aged care, disability support, or a similar community service roleExperience in or ability to learn to support Acquired Brain Injury and Dementia related disabilitiesA genuine passion for supporting and empowering vulnerable individualsStrong communication and interpersonal skills, with the ability to build rapport with clientsExcellent time management and organisational skillsA current First Aid and CPR certificate and a valid driver's licenceThe ability to work flexibly, including weekends and public holidays, as requiredAvailable for a minimum of 3 shifts per week.Certificate III/IV in Individual Support, Disability, or Community ServicesCurrent First Aid & CPRCurrent Blue Card & NDIS Worker Screening CheckValid driver's licence and reliable vehicleAbility to work with participants presenting challenging behavioursExcellent written and verbal communication and interpersonal skillsCommitment to dignity, respect, and independenceExperience working with ABI and/or DementiaKnowledge of SIL environments and NDIS frameworksKnowledge of working with Positive Behaviour Support (PBS) PlansMental health, trauma-informed care, or dual-diagnosis experienceWhat we offerAt Swan Healthcare, we are committed to providing a supportive and inclusive work environment.
