Overview
Afterhours Carer Services Navigator role at Wellways Australia.
This part-time position (24 hours per week) is based in Campbelltown, NSW and Upper Mount Gravatt, QLD.
Salary: SCHADS level 4B ($44.58-$47.97 ph) + Super + Salary packaging benefits.
The role provides support to carers seeking information through Carer Gateway Monday – Friday 8pm–8am, weekends and public holidays 24 hours.
The Carer Gateway Services have been created in the context of national reforms and represent an exciting new era for the delivery of innovative carer services.
This role is responsible for triaging carers contacting Carer Gateway, and where appropriate, implementing end-to-end services including intake, registration, assessment and planning and emergency respite care for carers who are experiencing crisis and carer burnout to access urgent support.
Responsibilities
Triaging carers contacting Carer Gateway.
Where appropriate, implementing end-to-end services including intake, registration, assessment and planning.
Providing emergency respite care for carers in crisis and burnout to access urgent support.
Qualifications
Diploma qualifications in a relevant field, with experience in community services, allied health or carer/family peer positions.
Experience in supporting individuals experiencing crisis and/or complex supports.
Right to work within Australia.
Current valid Driver's Licence and the ability to undertake some travel.
NDIS Worker Orientation Module Certificate (free online course).
Satisfactory National Police Records Check.
NDIS Worker Screening Check.
Working with Children's Check (Employee/Paid).
