Overview
We are looking for a Client Intake and Assessment Practitioner to join our Business Excellence team in Cannon Hill. You will be the first point of contact for our Home and Community enquiries, supporting clients to achieve their goals through access to Endeavour Foundation services.
Reporting to the Business Improvement Specialist, you will analyse disability assessments, conduct screenings, and create tailored support plans to ensure clients are matched with the right services based on their needs and funding. You will also contribute to initiatives and activities that drive growth, boost customer retention, build community awareness, and create pathways for people with disabilities to access Endeavour Foundation services across Qld, NSW and Vic.
Responsibilities
Understand and assess a client's support needs, confirm functional capability, and ensure staff and leaders have appropriate training to meet those needs.
Support a culture of person-centred practice that places clients, families, carers and guardians at the forefront of all activities.
Provide a customer experience in line with Endeavour Foundation values and behaviours, managing day-to-day relationships with clients, their families and external referral organisations.
Foster collaboration with regional site leadership teams, supporting co-creating and co-delivery across the business.
Use knowledge of the NDIS and disability sector to match clients to services that align with their goals and maximise the use of Individual NDIS funding.
Assist in developing a pipeline of qualified leads to drive growth and sustainability of our core customer program and identify new market opportunities.
What You Can Bring To The Team
With a deep understanding of the NDIS and disability sector, you excel at building strong relationships and developing meaningful connections. Your ability to analyse and interpret complex disability assessments allows you to create comprehensive risk assessments, safety plans, and support strategies that meet individual client needs.
You Will Also Have
Degree in Social Work, Psychology, Human Services or a relevant field.
Minimum 4 years' experience in the community services sector.
Experience analysing support needs and creating comprehensive assessments to maximise NDIS funding opportunities.
Large volume case management experience in disability or support networks is highly desirable.
Excellent relationship management, interpersonal and communication skills with an engaging and inspiring communication style.
Strong analytical skills with the ability to collect, analyse and disseminate data to drive customer conversion and provide actionable insights for stakeholders.
Hold or be willing to obtain a Working with Children Check (Blue Card), NDIS Worker Screening Check (Yellow Card), National Criminal History Check (Fit2Work), and an open driver's license with comprehensive insurance.
