Support at Home Program - Job Description
As a Support at Home program team member, you will oversee a portfolio of clients and provide the necessary support for them to safely stay at home while working towards their goals.
The responsibilities of this role are varied and challenging. Each day brings new opportunities to make a positive impact on the lives of our clients.
Assess client needs and develop personalized care plans to meet those needs.
Provide direct support to clients in their homes, including assistance with daily living tasks, medication management, and transportation.
Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.
Requirements:
A relevant qualification (e.g., Diploma of Case Management and/or Community Services/Nursing/ or related field).
A current Working with Children Check.
A Victorian Driver's Licence.
