Support at Home Program - Job Description

As a Support at Home program team member, you will oversee a portfolio of clients and provide the necessary support for them to safely stay at home while working towards their goals.

  • Assess client needs and develop personalized care plans to meet those needs.
  • Provide direct support to clients in their homes, including assistance with daily living tasks, medication management, and transportation.
  • Collaborate with other healthcare professionals to ensure comprehensive care and support for clients.

Requirements:

  • A relevant qualification (e.g., Diploma of Case Management and/or Community Services/Nursing/ or related field).
  • A current Working with Children Check.
  • A Victorian Driver's Licence.
