About the Role

The Practitioner of the Integrated Family Services team provides case management support and services to vulnerable children and young people from pre-birth up to and including 17 years of age, and their families, who are likely to experience greater challenges because the child or young person's development has been affected by the experience of risk factors and/or cumulative harm and/or at risk of concerns escalating and becoming involved with Child Protection if problems are not addressed. The role reports to the Integrated Family Service Team Leader and indirectly to the Children and Family Services Manager. Children and Family Services programs/services operate across Victoria and Tasmania and are funded by the Department of Health and Human Services, Victoria and/or Communities Tasmania.

The role is responsible for undertaking assessments of the needs of children, young people and their family, developing Child and Family Action plans by partnering with the family and other services to provide an integrated service within the Best Interests Case Practice Framework. Delivery of service occurs primarily through ongoing implementation, review and progress assessment of the needs of the client. The aim of the program is to deliver evidence informed best practice programs and services, as directed by the Leadership Team, in line with Baptcare strategic objectives for the Family and Children's Services teams in Victoria.

Key Duties

Deliver services in an accountable, professional manner and with superior customer service, in accordance with legislation, guidelines and duty of care requirements. This includes but is not limited to the Children, Youth and Families Act 2005 (Vic), Child Safe Standards, the Carer Recognition Act 2010 (Cwlth) and 2012 (Vic), the Charter of Human Rights and Responsibilities Act 2006, Children, Young Persons and their Families Act 1997 (Tas) and Work Health and Safety requirements.

Provide, monitor, assess and manage a case load that provides parenting support, skill development, referrals and community supports and services in line with the Best Interest Interests Framework

Plan, develop and monitor the implementation of evidence informed interventions.

Undertake responsibility to prepare and submit a range of high quality written reports to internal and/or external stakeholders as required within specified timeframes.

Maintain, review and report of case load and service delivery addressing service improvements with support and guidance to ensure full compliance, safety guidelines, targets and high standards of record keeping are being maintained in line with Baptcare legislative and contractual requirements.

Plan, facilitate, deliver, evaluate and report on family related groups as required.

Plan, co-ordinate, assist and administer the operation of a multi-functional program, including reporting in liaison with management

Engage with the day-to-day operations of the team including caseload management, performance management, professional development with guidance and support as required.

Maintain updated knowledge of and work within Baptcare and DHHS policies and procedures.

Support and maintain effective positive relationships within Baptcare and externally.

Support and assist, as required, operational and strategic relationships, planning and initiatives within Baptcare.

Develop, maintain, support and assist effective relationships with DHHS and other funding bodies.

Actively participate in relevant networking groups, meetings, forums.

Demonstrate a commitment to ensure risk management and monitoring obligations are fully met.

Demonstrate a commitment to service provision that meets all legislation, Government and Baptcare policies, procedures -as well as Funding and Service Agreements.

Demonstrate a collaborative and professional culture within the team and Baptcare to ensure quality service provision to clients and carers.

Demonstrate an understanding of policy and procedures with support and guidance from Team Leader.

Demonstrate a commitment to service delivery meeting evidence informed best practice guidelines, DHHS standards, QIC standards and Child Safe standards.
