**About Monash Health**

Working alongside a clinical and non clinical team, the AOD worker will provide an addition to the HOPE functions with AOD'S holistic approach to care. Working in line with Monash guideline and procedure, the role of the AOD clinician is to undertake holistic assessment of needs, treatment planning, monitoring, advocacy and linking of the consumer with appropriate community support services.The AOD- Psychosocial support role is an addition to the HOPE team approach to comprehensive care. The AOD - Psychosocial clinician is required to provide the best care to our communities. Comprehensive AOD care means that all aspects of a person's needs are considered when providing that care.

**About HOPE**

The (HOPE) initiative provides psychosocial support for people who are identified as having presented with a suicidal or self-harm attempt, or suicidal or intentional self-harm ideation. The initiative aims to provide follow-up care and support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support. The aim of the initiative is to prevent avoidable suicide attempts and reduce the number of completed suicides by providing targeted psychosocial support services to a cohort of people in an assertive manner in the community for up to 3 months from the time of discharge

The Clayton Hospital Outreach Post-suicidal engagement (HOPE) program provides services to individuals aged 18 years onwards following a suicide attempt or suicidal ideation. Services are provided for approximately 3 months and are predominantly psychosocial or peer, with limited supports from clinical services as required. As an AOD/psychosocial clinician at the Clayton HOPE program you will hold a small caseload of clients with dual diagnosis and will be expected to draw on your AOD and clinical skills.

This position will draw on your knowledge in mental health legislation and policy. Demonstrating your expertise in AOD/psychosocial and clinical skills, you will undertake education and informative collaboration with the Community Mental Health Team, key stakeholders such as other mental health services both inpatients and community, drug court, housing services, counselling and observance of links with other AOD services throughout the metropolitan Melbourne. Client contact may occur at the clinic, via teleconference or at their own accommodation as required.

The position requires an individual with skills which will enable them to liaise closely with a number of Inpatient and community agencies to ensure best practice care is provided. This will ensure a comprehensive assessment, collaborative treatment design and appropriate referral of consumers presenting to the team

It will be based at our new purpose designed Community Hub in Atherton Rd Oakleigh under the direction of the Manager Adult Community Mental Health.

The position involves Monday - Friday daytime roster. Clayton Community Mental Health Service is located close to the CBD and the South East sector. It is an autonomous role with the balance of working in a team environment. Access to high quality clinical supervision is offered.
