Service Support Officer Care Navigation - What's involved in the role?
The role of the Service Support Officer (SSO) Care Navigation works within HammondCare's Mission and Mission in Action to ensure that all home care services are tailored, personal, and delivered to the highest standards.
The SSO handles a broad range of administrative activities for our teams, potential clients, clients, representatives, and contractors, both locally and centrally.
This key role supports various areas across HammondCare at Home (HCAH), which operate both locally and centrally.Care NavigationThe administrative activities in HammondCare at Home are essential to daily operations and planning, supporting care teams, client care, potential clients, and contractors involved in providing and planning care services.Key Focus Areas:Provide reliable and consistent administrative support to colleagues, potential clients, clients, their representatives, and contractors across our Care Navigation Service, Partner Services, and Areas/Regions.Collaborate with colleagues to ensure best practices and consistent workflows to meet regulatory, service, and care standards.Support positive communication, collaboration, and engagement within the team and with internal and external stakeholders.Support stream activities and assist with other streams as needed for seamless administration across HCAH.What do you need to apply?
Essential:Commitment to HammondCare's Christian Mission, Motivation, and Mission in Action.Previous experience in an administrative role or similar.Excellent telephone etiquette and communication skills.Ability to engage professionally and courteously with clients and customers over the phone.Proficiency in managing inbound and outbound calls, inquiries, and providing information effectively.Strong verbal communication skills for clear and concise articulation over the phone.Effective organisational skills, including time management and prioritisation.Knowledge of compliance requirements within the Aged Care sector.Initiative, analytical, and problem-solving skills.Strong administrative and computer skills.Ability to work independently and as part of a team.Empathy and patience.Commitment to customer service excellence to ensure exceptional service delivery.Willingness to complete mandatory MACLearning requirements via the MACLearning portal (Care Navigation Service only).
