**The Position**

**Our innovative service model focuses on delivering the best clinical outcomes for clients. As an experienced Dietitian with Empower, you will have the opportunity to work with a variety of patients across home care in the Northern and Southern suburbs of Perth.**

**This role is a 12-Month Maternity Leave Cover.**

**You will**:

- Complete comprehensive nutritional assessments for residents and for clients in their own home through home care packages/NDIS.
- Develop personalized nutrition plans and goals
- Support the dietary requirements of individuals with difficulty swallowing and chewing, bowel and intestinal issues and other complex issues.
- Provide weight management, poor appetite and dysphagia interventions
- Manage food intolerances and allergies
- Provide education on diabetes management
- Educate clients and their families to optimise their ability to live healthy lives within the community
- Work holistically with other allied health professionals to provide a multidisciplinary care plan
- Liaise with service providers to ensure effective communication
- Ensure caseload and documentation are timely and completed at a high standard

**You will be provided with support from our home care team who will schedule these appointments and plan an efficient route.**

You will have an enthusiastic and proactive attitude, be outcome focused and dedicated to delivering an exceptional service to our clients.

You must have:

- Bachelor of Nutrition and Dietetics (or equivalent)
- Experience working within home care, aged care and NDIS settings
- Membership with Dietitians Association of Australia with Accredited Practicing Dietitian status
- Police Check Clearance
- Excellent verbal and written communication skills
- Strong time-management skills and attention to detail
- Ability to work independently and as part of a team
