- You will be the first point of contact for NDIS participants, families and Support Coordinators who are seeking information about New Horizons or who have chosen us to provide services for them under an NDIS plan._

**Based in the Central Coast (Wyong)**, the role is a Mental health Community based role as a **Full-Time - Support Worker **with rotating shifts providing the highest quality in high needs / complex care Mental health and disability services to the community.

The hours of work vary from** weekday shifts, evening and weekend shifts.**

**You will report to the Team Leader in our Inclusion team.**
- These positions support the delivery of Community based recreational programs both group based and individualized as well as assistance with daily life and self care._

**The Day to Day**
- Providing a person centred approach to help customers achieve their goals and aspirations
- Contributing information for the development of Customer support plans
- Providing support to customers with living skills, transport, social activities, medication and other supports as required
- Keeping customer records up to date on a daily basis using our on line systems
- Developing and maintain professional relationships and networks with customers, families, caregivers and key stakeholders
- With support & guidance from the Team Leader, seek out innovative ways to enhance the customer journey
- Managing some challenging behaviours

**What you need to have**:

- Appropriate certificate level qualification in Disability or Community Services, OR equivalent relevant experience in the human services industry
- Valid NSW Drivers C class licence, with ability to drive vehicles in licence class, including large cars and vans.
- Must be fit to undertake the physical requirements of customer support, including the capacity to lift 10 kilograms unassisted
- Intermediate skills in Microsoft Office, other common systems
- Passion, Respect & Integrity

**Ideally you will also have**:

- Experience in personal care, aged care, manual handling, disabilities or AIN (assistance in nursing) would all be highly regarded.
- Advanced Solid knowledge of services and programs under NDIS
- Understanding of Person-Centred Practices.
